Program Summary

In some cases, the plan requires you to first try one drug to treat your medical condition before they will cover another drug for that condition.

Certain prescription drugs will have maximum quantity limits.

Your provider must get prior authorization from Medco Medicare Prescription Plan® for certain prescription drugs.

Covered Part D drugs are available at out-of-network pharmacies in special circumstances including illness while traveling outside of the plan's service area where there is no network pharmacy. You may also incur an additional cost for drugs received at an out-of-network pharmacy. Please contact the plan for details.

MEDCO By Mail, Medco's mail-order pharmacy, gives you the convenience and savings of having your long term medications delivered right to your door. Your prescriptions will go through multiple live and automated checkpoints to ensure accuracy and to help protect you from harmful drug interactions each time they are filled.
Mail order is most appropriate for drugs that you take on a regular basis for chronic or long-term medical conditions. When you use Medco by Mail, Medco's mail-order pharmacy, it can help lower our prescription costs in order to delay reaching the Coverage Gap (also known as the "doughnut hole"). You can get a 90-day supply of our long-term generic medications and pay only your plan's mail-order copayment.
**Note: The rates for the prescription drug plan are included in the rates reflected under the medical plan page as the medical and prescription drug plan are a combined offering and cannot be separated.

Does my plan cover Medicare Part B or Part D
drugs? |
|
Drugs that are covered under Medicare Part B
as prescribed and dispensed are not covered.
Generally, we only cover drugs, vaccines,
biological products and medical supplies
that are covered under the Medicare
Prescription Drug Benefit (Part D) and that
are on the Medco Medicare Prescription Plan® |

Where can I get my prescriptions? |
|
More than 50,000 pharmacies belong to the
Medco Medicare Prescription Plan® network of
pharmacies. You must use a network pharmacy
to receive plan benefits. The Medco Medicare
Prescription Plan® may not pay for your
prescriptions if you use an out-of-network
pharmacy, except in certain cases. Please
note that the pharmacies in our network can
change at any time. You can
find a network pharmacy online or call
Customer Service for an up-to-date list.

What are the benefits of using mail order? |

Medco By Mail, Medco’s mail-order
pharmacy, gives you the convenience and
savings of having your long-term medications
delivered right to your door. Your
prescriptions will go through multiple live
and automated checkpoints to ensure accuracy
and to help protect you from harmful drug
interactions each time they are filled.
You’ll also have access to Medco’s
specialist pharmacist who offer you
personalized care and enhanced safety
because they have received additional
training in the medications that treat a
single condition, such as diabetes, heart
disease, or cancer, to help ensure your
medications are working best for you.

How do I access Medco By Mail? |
|
You can use the mail-order service to
fill prescriptions for most drugs on the
formulary list. Mail order is most
appropriate for drugs that you take on a
regular basis for a chronic or long-term
medical condition. Drugs that cannot be
purchased through mail order include
medications with limited distribution and
compound medications. It’s also more
appropriate to use a retail network pharmacy
for drugs use for a short period of time (1
month or less) and drugs needed immediately
for the treatment of a severe medical
condition.

How do I save money using mail order? |
|
When you use Medco by Mail, Medco’s
mail-order pharmacy, it can help lower your
prescription costs in order to delay
reaching the Coverage Gap (also known as the
“doughnut hole”). You can get a 90- day
supply of your long-term generic medications
and pay only your plan’s mail-order
co-payment. Your long-term medications will
be delivered right to you, with free
standard shipping.

What should I do if I have other insurance? |
|
DALRC Retiree Benefit Trust is only
sponsoring this plan. If you elect to enroll
in, or retain, any other Prescription Drug
covereage, the subsidy you receive and your
DALRC Retiree Benefits may be impacted. |

I've been hearing a lot about the "donut
hole." What is this? |
| The
"donut hole" or coverage gap is the segment
where you are responsible for paying the
full cost of your medicines. |

Do any of the plans offered by the DALRC
have a "donut hole"? |
| Yes.
The Opal Enriched Low Option Plan although
lower in monthly premium does have a "donut
hole" or coverage gap. This means that once
you reach the third segment of the plan you
have to pay 100% of your prescription costs
until your out-of-pocket costs reach $4,550
(for 2009). |

If I choose the plan with a "donut hole" how
will I know when I've reached the "donut
hole"? |

Medco Medicare Prescription Plan®
sends you monthly statements that will track
your total drug spend, so you always know
where you stand. If you have any questions,

What can I do to get additional assistance
to help me pay for my premium? |
|
You may be able to get extra help to pay
for the monthly premiums, annual
deductibles, and co-payments related to your
Medicare prescription drug costs. Medicare
provides this program for people who have
limited income and resources. If you
qualify, this assistance will count toward
your out-of-pocket costs.
